Form-76 Eligibility Review & Equivalency of Training (EOT) Registration
1. Agreement to Terms
By submitting payment or registering for any GovSource service, you acknowledge that you have read, understood, and expressly agree to be legally bound by these Terms and Conditions.
These terms apply to all services including Form-76 Eligibility Review, EOT registration, diagnostic exams, and related administrative services.
2. Immediate Commencement of Services
Upon submission of payment, GovSource immediately begins administrative and processing services. This includes opening your eligibility review file, assigning a tracking ID, initiating document verification workflows, and allocating staff resources.
For EOT registrations, your training seat is reserved at the time of payment.
Because services and resource allocation begin immediately, all payments are non-refundable. All sales are final.
3. No Refund Policy (Strict Enforcement)
All payments made to GovSource are final and non-refundable.
This includes:
- Form-76 Eligibility Review fees
- EOT course registrations
- Diagnostic exams
- Administrative and processing services
No refunds will be issued under any circumstances, including:
- Change of plans
- Failure to complete documentation
- Ineligibility determinations
- Personal or financial circumstances
- Scheduling conflicts
- Failure to attend
By registering for an EOT course, you acknowledge that your seat is reserved immediately upon payment and removed from available inventory. Once reserved, this seat cannot be reassigned or resold, and therefore the purchase is non-refundable.
4. Form-76 Eligibility Review
GovSource provides administrative support for eligibility review processing.
GovSource does not guarantee:
- Approval of Form-76
- Certification eligibility
- Employment with any agency
Applicants are solely responsible for submitting accurate, complete, and properly documented information.
Failure to do so may result in delays or closure of the file without refund.
5. EOT Course Registration Policy
Rescheduling Policy
EOT registrations may be rescheduled only if:
- The request is submitted more than 14 days prior to the course start date
- The request is submitted in writing
Approved reschedules will be moved to a future available course.
Within 14 Days of Course Start
- Rescheduling is not permitted
- Registration is forfeited
- No credit or refund will be issued
Failure to Attend (No-Show)
Failure to attend a scheduled course results in:
- Immediate forfeiture of registration
- No reschedule or refund
Reschedule Limitation
Once a reschedule request has been approved, the registration remains active and is not eligible for a refund under any circumstances.
6. Course Participation Requirements
Participants must:
- Meet all eligibility requirements
- Arrive on time and prepared
- Follow all safety, training, and instructor directives
GovSource reserves the right to remove any participant for:
- Safety violations
- Misconduct
- Failure to comply
Removal from a course does not qualify for a refund or reschedule.
7. Scheduling and Operational Changes
GovSource reserves the right to:
- Reschedule courses
- Modify course locations, instructors, or formats
If GovSource reschedules a course:
- Participants will be placed into a future available course
- No refunds will be issued
8. Limitation of Liability
GovSource is not responsible for:
- Travel expenses
- Lodging costs
- Lost wages
- Any indirect or consequential damages
Participation in any GovSource service or training is voluntary and at the participant’s own risk.
9. No Guarantee of Employment or Certification
GovSource provides training and administrative support services only.
GovSource does not guarantee:
- Employment with any law enforcement or corrections agency
- Passing of the State Officer Certification Exam (SOCE)
- Issuance or approval of Form-76
10. Payment Disputes and Chargebacks
By completing payment, you acknowledge and agree that services begin immediately and that your purchase is non-refundable.
You agree not to initiate a chargeback or payment dispute with your bank or payment provider for services rendered.
In the event of a dispute, GovSource will provide evidence of:
- Your acceptance of these Terms and Conditions
- Immediate service initiation
- Seat reservation and resource allocation
Any attempt to reverse payment may result in:
- Immediate termination of services
- Ineligibility for future GovSource programs
- Referral to collections for recovery of funds and associated fees
GovSource reserves the right to contest any chargeback with supporting documentation.
11. Electronic Signature and Agreement
By selecting the Terms and Conditions checkbox and submitting payment, you provide an electronic signature that is legally binding and equivalent to a handwritten signature.
12. Proof of Service Delivery
GovSource maintains records including:
- IP address
- Timestamp
- Submitted forms
- Payment confirmation
- Communication logs
These records may be used as evidence of agreement and service delivery in the event of a dispute or legal proceeding.
13. Non-Transferable Services
All services are non-transferable and may only be used by the registered applicant.
14. Communications Consent
By registering, you consent to receive communications from GovSource, including:
- Phone calls
- SMS/text messages
These communications are part of the service process.
15. Governing Law
These Terms and Conditions are governed by the laws of the State of Florida.
